Recurring Donations Import
This how-to will show you the fields to be included in the Import Template with Picklist Values for each Recurring Donation. The Recurring Donation Name should be included on the Opportunity Record.
Template, Part I
I will attach the template I've created to the email I send you with this. Below are the picklist values for the Picklist fields.
Recurring Type Field

You can add to these picklist values as needed. Setup > Recurring Donations Object > Fields and Relationships
Status
Not sure what "Failing" does, but it seems to indicate a donor who is not fulfilling their commitment to the Recurring Donation?
Status Reason
Active Schedules
These are found in the right-hand column
Upcoming Installments
Shows the dates fo the next 12 installments.

Other considerations
- You've disabled Recurring Donation Name Formatting. You may want to reconsider that decision. You can choose what goes into the naming convention and this feature provides a certain level of name conformity that makes search easier.
- Don't forget to add the Recurring Donation name to the Opportunity Records that come in that are associated with the Recurring Donation.
- Also be sure to add the Campaign as well.
- Your Opportunity Record Type is C&P Online. You may want to change that to Recurring Donation Record type, or some other, less app specific name.
